Enrollment Information

In order to enroll at Canyon Rim Children’s Center, your child must be between the ages of 2-6 years old. Application forms are available at the Center office. Prior to acceptance, parents must bring the child to the Center to meet the Director and the child’s potential teachers. CRCC does not discriminate against any race, color, religion, sex, national origin, or ancestry. This applies to the hiring of staff and the applications of our students. Children are accepted into the center on a space-available basis. In order to be admitted for attendance, a completed registration packet must be submitted at or before the time of enrollment. Each packet contains a Parent Handbook, Immunization Forms, Physician’s Form, Parent and Center Agreement, Tuition Policy Agreement, and other important forms to be read and signed by the parent or guardian. Once all required forms are completed, and the registration and initial tuition fees paid, your child may begin attendance at CRCC. The following forms must be current and are to be maintained in each child’s folder:

• Tuition Agreement
• Parent/Center Agreement
• ID/Emergency Information
• Most recent Registration Card
• Medical Consent Form Physician’s Report
• Immunization card
• Personal Rights (The Children)
• Parent’s Rights
• Developmental Profiles & any Incident Reports
