Form F01 – Click Here for Credit Card Authorization
CRCC offers “EASY PAY”. This allows you to authorize us to regularly (biweekly or monthly) charge your tuition payments and related fees to a Visa or Master Card.

Form F02 – Click Here for Credit Card Authorization CANCELLATION
If you have placed a Credit Card Authorization on file with CRCC and you wish to cancel it, this form must be completed and turned in to the office.

Form F03 – Click Here for Notice of Withdrawal
This form is to be completed and turned into the Director of CRCC at least two weeks prior to the effective date of withdrawal.

Form F04 – Click Here for Children’s Pictures Personal Use Agreement
CRCC requires any individual not an employee of CRCC taking or recording images of children on the grounds of CRCC to sign this agreement.

Form F05 – Click Here for Sunscreen Permission Slip
Parental permission is required for CRCC employees to apply sunscreen to enrolled children.

Form F06 – Click Here for Nebulizer Authorization Form
The State of California requires this form to be completed and on file in order for a CRCC employee to be permitted to administer an inhaled medication. Please read the instructions carefully.

Form F07 – Click Here for Injured/Recovering Child Instructions Form
This form must be completed and on file for any child returning to CRCC following an injury or a medical procedure requiring limitations or restrictions.

Form F08 – Click Here for Prescription Medicine Permission Form
This form must be completed and on file in order for a CRCC employee to administer prescription medications to a child in our care.
